Print out and complete the application, the disclosure statement and the terms and condition agreement. (This information can also be typed directly on the PDF version of the form)
Make a copy of the proper address and social security verification as indicated on the sample page of the enrollment form. Please make sure copies are legible and clear as your application can not be processed without them.
Write a check or money order payable to Third Eye, Inc
Enclose the application, disclosure statement, terms and condition agreement, address and social security verification, check or money order, and mail it to:
